June 2014 Pharmacist Licensure Examination Passers

The Professional Regulation Commission (PRC) released the list of passers of the Pharmacist Licensure Examination given by the Board of Pharmacy in the cities of Manila, Baguio, Cebu and Davao this June 2014. They announced that 1,525 out of 2,779 passed the board exam.

Registration for the Issuance of Professional ID and Certificate of Registration

Date: July 3 and 4, 2014
Requirements:

1. Duly accomplished Oath Form or Panunumpa ng Propesyonal
2. Current Community Tax Certificate (cedula)
3. 1 piece passport size picture (colored with white background and complete name tag)
4. 2 sets of metered documentary stamps
5. 1 short plastic envelope with name and profession

Payment:

1. Initial Registration Fee of P600 
2. Annual Registration Fee of P450 for 2014-2017
